In fact, there is reason to celebrate. Not only is it almost ten years to the day since Dr. Martin Gruber came back to Ellmau. It's also Lisbeth's birthday. Because his patient Hanna Hofer received the donor heart from Michael's wife, but has been suffering from severe depression ever since. Martin arranges a meeting between the two and hopes that this will give Hanna some courage to face life again. The plan seems to be working, the chemistry between the two is right from the start, they form a tender bond. But Hanna's latest laboratory results point to a non-specific inflammation and an incipient rejection reaction of the new heart. Michael doesn't want to admit his feelings for the young woman who carries the heart of his great love within her, and Hanna loses her newly found joy in life. She's getting worse and worse. With the help of his friend Kahnweiler, Martin feverishly searches for the source of the infection. Can he save Hanna?

Martin Gruber seems ready for his "new life". He has made a truce with Anne, left the Gruberhof and has moved into a small apartment. At a meal together, the situation with the teenager escalates. Although Lukas later apologizes to Martin at his mother's urging, his joy is limited when Rike suggests spending a few days together as a couple. Martin's friend Kahnweiler, on the other hand, would be happy if his Vera would finally get in touch again. She's been hiding with a friend for a week because she wants to clear up a few things. Does that mean the end for the two?

The case of his patient Katja Weiss challenges Dr. Martin Gruber's full attention. For some time she has been suffering from headaches and numbness in her hands. Sebastian Lechner, a withdrawn man suffering from Asperger's syndrome, for whom Katja regularly does small errands, is alarmed and sends the young, single mother to the doctor. The finding is quick: A harmless cyst in the head seems to be responsible for the neurological deficits. Because if he doesn't operate on her himself, there is a risk of severe paralysis. But Lechner can't get out of his skin. Does Martin manage to convince him - for Katja's sake? Hans Gruber has been boiling inside ever since he found out that Susanne has something to do with Tim. He becomes even more angry when he realizes that the surveyor is spending more and more time with his daughter. After Susanne, does he also lose little Sophia? Rike's family, on the other hand, is exactly Martin's problem. He likes her, but doesn't know how the three of them should work with her son Lukas. After daughter Lilli gently urges him to stop shirking the truth, he arranges to meet Rike to break up. But when Lukas finds out about the breakup, he freaks out completely and disappears without a trace. Martin goes off on his own and finds the boy in a new climbing area. A fight ensues, Martin loses his balance in the scramble and slides down a steep slope. But instead of going for help, Lukas, who is in the middle of a manic phase, simply turns around, leaving the badly injured and unconscious Martin alone.

Luck in disguise for Martin Gruber. Because although Lukas left him alone and badly injured after his fall on the mountain, thanks to Anne's efforts, Martin is found. But while he is still in the hospital, he closes in on his family and rejects Lisbeth's offer to come back to the farm to recover. The situation with Anne is still easy to deal with. Although he is still visibly shaken, Martin throws himself into his work as passionately as ever. One case particularly affects the Bergdoktor. Since the death of her mother, 16-year-old Alina has been living with her father Daniel Förster, his wife Petra and their son Finn. When the depressed girl wants to end her life with pills, the consequences could hardly be worse. Because her little half-brother Finn accidentally poisoned himself with the substances when he drank from Alina's glass. Both children then collapse with severe organ damage. The only chance of salvation is a liver transplant, and as soon as possible, because Alina and Finn's condition is deteriorating rapidly. Only father Daniel comes into question as a donor, but the bad news from Martin and Kahnweiler hits him like a punch: he can only donate part of his liver to one of his children, so he has to decide whether he wants to save the life of Alina or Finn. His wife Petra asks him to decide for her son. But he doesn't want to let Alina die either, especially now that they are about to finally build a father-daughter relationship. When Daniel is about to do something stupid, Martin has an idea. Is there hope for both children after all?

Actually, Martin Gruber should finally take it easy after the recent events, but the mountain doctor just can't get any rest. She first moves back to her parents, although she hasn't had any contact with them for quite some time because they disapprove of her relationship with Thomas. Martin knows: You shouldn't pressurize amnesia patients and confront them with the forgotten events too early. Because Thomas is now almost a stranger to Lisa, he asks him to keep his distance for the time being until the young woman remembers herself. But that can take weeks or months.
